Why the loops hold

Four rules. No exceptions.

The referee stands outside the field. The checker, the budgets and the permissions sit outside anything the loop can change. A rule that lives only in a prompt gets optimized away. A rule built into the system holds.
Measure before it goes live. A change goes live only when measurement shows the problem is fixed and nothing else got worse.
The written core grows in small steps. It is never rewritten in one go, only extended in small, tested steps. That keeps the loop on course.
Failure is data. Every failed attempt is kept. Negative results are the cheapest way to make the next loop smarter.
